Non-Surgical Calf Reduction in Seoul
Yaan Clinic · Gangnam, Seoul
Non-surgical calf reduction slims bulky calf muscles without an incision. At Yaan Clinic in Gangnam, Seoul, doctors first identify your calf type, then choose between a one-hour selective nerve-blocking procedure, a targeted calf slimming injection for the lower calf and ankle, or calf Botox with genuine Xeomin. It suits people whose calves look thick because of muscle rather than fat.

Non-surgical calf reduction is a group of treatments that make the lower legs look slimmer by reducing the size or activity of the calf muscles instead of cutting tissue. At Yaan Clinic in Gangnam, Seoul, calf contouring has been part of the clinic's body-contouring work for more than 14 years, and every plan starts with an examination of the muscle structure of your legs.
This page explains the three calf options Yaan Clinic offers, how they differ, what recovery looks like and how to plan treatment around a trip to Korea.
What is non-surgical calf reduction?
Non-surgical calf reduction slims the calves by weakening or shrinking the gastrocnemius and soleus muscles through injections or nerve-targeted treatment, with no incision. It is designed for calves that look large because of muscle bulk, not fat.
Many people develop prominent calves from genetics, sports, walking in heels or simply from the way they stand. Exercise and dieting usually do not help, because the bulk is muscle. Surgical calf reduction (muscle resection) exists, but it involves a scar and a longer recovery. The non-surgical options at Yaan Clinic aim for a slimmer, straighter leg line with a small injection mark only.
Which calf reduction options does Yaan Clinic offer?
Yaan Clinic offers three approaches: a calf reduction procedure using selective nerve blocking, a calf slimming injection for the lower calf and ankle, and calf Botox with Xeomin. Your doctor recommends one, or a combination, based on your calf type.
1. Calf reduction procedure (selective nerve blocking)
This non-incisional procedure selectively blocks branches of the motor nerve that drives the gastrocnemius muscle, so the targeted part of the muscle gradually shrinks. Yaan Clinic uses the Electrothermal 20S device, which targets calf motor nerves, and can add its Body Sculpture Injection to refine the ankle line. An AccuVein infrared vein finder helps the doctor see vessels before treatment. The procedure takes about one hour under local anesthesia.
2. Calf slimming injection
The calf slimming injection selectively reduces calf muscle fibers after a nerve examination. Yaan Clinic describes a "Click Line" technique for the gastrocnemius (upper calf) and a "Click Line Injection" for the soleus (lower calf near the ankle). It is aimed at thick lower calves, calves that look wide when flexed, asymmetry and a heavy ankle line. Downtime is little to none.
3. Calf Botox
Calf Botox relaxes overdeveloped calf muscles with botulinum toxin. Yaan Clinic uses Xeomin (MERZ, Germany) and is an official MERZ Key Doctor clinic. Read more on our Botox treatments in Seoul page. Botox is the gentlest entry point, but results are temporary.
What calf type do I have?
Yaan Clinic classifies calves into three types: muscle-type, drumstick-type and log-type. The type shows which muscles are enlarged and decides which treatment is likely to work best.
| Calf type | Muscles involved | How it looks |
|---|---|---|
| Muscle-type | Medial (inner) gastrocnemius | A bulge on the inner upper calf, often more visible on tiptoe |
| Drumstick-type | Medial and lateral gastrocnemius | A rounded upper calf that narrows sharply toward the ankle |
| Log-type | Gastrocnemius and soleus | A straight, thick calf with little ankle definition |
Fat and skin also play a role. If part of the thickness is fat rather than muscle, your doctor may discuss Yaan's fat-dissolving Body Sculpture Injection for the ankle and lower leg.
Calf reduction procedure vs calf Botox vs surgery: which is better?
For lasting change without a scar, the nerve-blocking procedure is usually preferred; Botox suits people who want a temporary, low-commitment option; surgery leaves a scar and carries more risk. The right choice depends on your calf type and goals.
| Calf reduction procedure | Calf Botox (Xeomin) | Surgical muscle resection | |
|---|---|---|---|
| Incision | None (small injection mark) | None | Yes, about a 2–3 cm scar |
| Treatment time | About 1 hour | Short | About 2 hours |
| Anesthesia | Local | Usually none or topical | Surgical anesthesia |
| When results show | Muscle shrinks from 3–4 weeks; clearest at about 6 months | Gradual over weeks | After surgical recovery |
| How long it lasts | Semi-permanent | Repeat every 4–6 months | Permanent |
| Main risks | Temporary discomfort; results vary by muscle | Temporary weakness; effect wears off | Nerve and blood vessel injury, scarring |
Figures for the procedure and resection are Yaan Clinic's own; Botox timing is typical and your doctor will confirm it at consultation.
How long do calf reduction results last?
Results from the selective nerve-blocking procedure are described by Yaan Clinic as semi-permanent. Calf Botox usually needs repeating every 4–6 months to keep the effect.
After the procedure, the targeted muscle begins to atrophy at around 3–4 weeks, continues to reduce over the following month and is most noticeable at about six months. Muscle response differs from person to person, and some muscle volume can return in some patients. Yaan Clinic offers re-treatment when it is needed, which it notes many clinics do not.
Is calf reduction painful? What is recovery like?
The procedure is done under local anesthesia, so most people feel pressure and mild discomfort rather than sharp pain. Expect some soreness on the day, then a gradual return to normal over the following days.
Calf slimming injections and calf Botox typically involve little to no downtime, and most people walk out and return to daily activities straight away. Your doctor will give you a timeline for exercise, running and high heels. Yaan Clinic's FAQ notes that heels and exercise are possible with planning.
Good to know: Selective nerve blocking aims to slim the calf while reducing the risk of gait disturbance, because only selected branches are treated. Like any medical procedure, results and recovery vary, and no clinic can guarantee a specific size reduction.
Who is a good candidate for non-surgical calf reduction?
Good candidates have calves that look thick or prominent because of muscle, want a slimmer leg line without surgery, and are generally healthy. Uneven calves and a heavy ankle line can also be treated.
- Calves that bulge when you stand on tiptoe or wear heels
- Calves built up by sports or exercise
- One calf noticeably larger than the other
- Legs that look bowed or knock-kneed because of calf shape
- A thick lower calf with little ankle definition
- People who have had calf Botox before and want a longer-lasting option
Treatment may not suit you if you are pregnant or breastfeeding, have a neuromuscular disorder, or rely on maximum calf power for competitive sport. Your doctor will review your health history at consultation.
What happens on the day?
- Consultation and calf assessment – the doctor examines your calves standing and on tiptoe to identify your calf type and which muscles are enlarged.
- Treatment plan – you agree on the procedure, injection, Botox or a combination, plus any ankle refinement.
- Marking and vein mapping – target points are marked and AccuVein is used to visualize vessels.
- Local anesthesia and treatment – the procedure takes about one hour; injections are quicker.
- Aftercare instructions – you receive guidance on walking, exercise and heels before you leave.

Frequently asked questions
Is non-surgical calf reduction permanent?
Yaan Clinic describes its selective nerve-blocking calf reduction procedure as semi-permanent. The muscle starts to shrink at 3 to 4 weeks and looks clearest at around six months. Some muscle volume can return in some people, and Yaan offers re-treatment if needed. Calf Botox, by contrast, usually lasts 4 to 6 months.
Will calf reduction affect how I walk?
The procedure selectively blocks only certain nerve branches to the gastrocnemius muscle, which aims to slim the calf while reducing the risk of gait disturbance. Most people walk normally right after treatment. Your doctor will explain what to expect for your calf type at consultation.
Can I exercise or wear high heels after calf reduction?
Yes, with planning. Yaan Clinic notes that exercise and heels are possible after treatment, but your doctor will give you a personal timeline. After calf injections or Botox, most people return to daily activities right away, while strenuous training may be paused briefly.
How many sessions of calf reduction will I need?
Most patients need one session of the calf reduction procedure. People with very developed muscles may need more. Calf Botox is repeated every 4 to 6 months if you want to keep the result. Your doctor will estimate this after examining your calves.
Can uneven calves be corrected?
Yes. Asymmetric calves can be treated by adjusting the area and amount treated on each side. Your doctor examines both legs standing and on tiptoe, then plans each side separately to bring them closer in shape.
I already had calf Botox. Can I switch to the procedure?
Usually yes. Many people try calf Botox first and later choose the nerve-blocking procedure for a longer-lasting result. Tell your doctor when you last had Botox and how much, so timing and dosage can be planned safely.
